Diabetes Gold package (type 2)
* Annual comprehensive diabetes assessment to include:
* Height/weight/BMI
* Diabetic foot assessment
* Blood pressure
* Blood testing to include assessment of blood sugar control along with other key indicators of heart and overall health and well-being.
* Further appointment at 6 month interval to monitor and advise on the management of your diabetes. This will include a further blood test to assess your blood sugar control.
* Annual flu jab
* 2 additional 20 minute appointment with our specialist diabetic nurse to be booked at a time of your convenience during the 12 month period.

Respiratory assessment
Full assessment of people with undiagnosed respiratory symptoms (including persistent cough, wheeze and/or unexplained shortness of breath). To include spirometry with reversibility assessment if necessary- approximately 1 hour consultation.

Cardiovascular assessment
Cardiovascular assessment for individuals previously diagnosed with any heart conditions to include blood pressure, ECG and blood testing. Followed by a consultation to discuss the findings with one of our chronic disease nurse specialists.
